The winner will show outstanding outcomes for patients or communities, compassion and professionalism, and a commitment to raising the standard of care.
Eligibility
Open to UK-based healthcare providers, practices, clinics, care organisations, health businesses and individual health professionals, with achievements within the last 12 to 24 months. Self-nominations and third-party nominations are both welcome.

Additional criteria
Submissions should include evidence of outcomes, patient or community feedback, and a testimonial from a colleague or partner. Patient confidentiality must be respected.
